邱瑋璇揭變體基因增乳癌復發率

乳癌是香港女性的頭號癌症殺手。每年有超過3,500宗新症,每17個女性當中就有1人有患病風 險。而近年來乳癌亦出現年輕化的跡象,有36.1%的患者年齡介乎20歲至45歲,情況值得關注。為更針對性幫助患者治療以及減低復發率,香港大學李嘉誠 醫學院病理學系臨床教授邱瑋璇一直致力研究乳癌的易感基因。經過3年的研究,發現病者的基因出現剪接變體BQ。由於高BQ的病者會對藥物產生抗藥性,她計 劃繼續研究針對BQ的單克隆抗體,從而能夠篩選高BQ的病人,為其找到例如標靶治療等最合適的治療方案。
邱瑋璇是今屆「裘槎優秀醫學科研者獎」的得主之一。她是乳癌分子遺傳學及病理生物學的專家,也是乳癌 易感基因(susceptibility gene)研究的先驅。於多年的研究歷程中,她亦特別把研究焦點放於香港的乳癌患者身上。她指出,不少患者都因基因突變和遺傳問題而患上乳癌。
邱瑋璇曾發表有關從華裔乳癌與卵巢癌患者身上發現易感基因BRCA1/2的論文,又研發可有效預測乳 癌輔助治療療效與抗藥性產生的分子標記。她解釋指,有效地找出分子標記,就能預測到乳癌的治療療效。同時,她指出乳癌細胞生長與雌激素受體相關,雌激素受 體是細胞內的蛋白質,可被雌激素激活。而荷爾蒙藥物「他莫希芬(Tamoxifen)」就是一種選擇性雌激素受體調制劑,可使乳癌死亡率降低25%至 30%。
研抗體針對性標靶治療
不過,不是每個病人都能有效借助「他莫希芬」而控制病情,約有一半病人會對此產生抗藥性。邱瑋璇指, 有病人體內會出現選擇性剪接(Alternative Splicing),而出現不同組合的外顯子(Exon)。她發現了一個NCOR2的基因全新剪接變體,將之命名為BQ。有高BQ的患者會對「他莫希芬」 產生抗藥性。這樣的話,癌症復發率會較高,癌細胞亦容易擴散及轉移。
因應獲獎後可暫時放下教學更專注於研究,邱瑋璇打算進一步研究BQ這個新發現的剪接變體如何導致「他莫希芬」抗藥性,亦會針對BQ而研發單克隆抗體,以為病人作出更針對性和準確的標靶治療。

ultrafast rendezvous to ISS
